The Vital Statistics Division of the Youngstown City Health District is responsible for:
Recording all births and deaths that occur in the City of Youngstown and Mahoning County, excluding the city of Struthers.
Issuing certified copies of birth and death certificates of those events that occurred in the City of Youngstown and Mahoning County, excluding the city of Struthers except for the years 1892 until 1932.
Keeping on file births and deaths that occurred in the City of Youngstown and Mahoning County from 1892 until the present; Struthers 1892 until 1932.
Certified copies of these certificates are provided on request and upon receipt of payment. A fee of $18.00 is charged for each certified copy. A certified copy is a copy of the original record and is required for any legal matter including passport, adoption, social security benefits, state identification cards, drivers license, and etc.
Birth or death certificates request may be made by:
Mail – when requesting a birth or death record include the name on certificate, date of birth or death, mother’s maiden name, father’s name, money order, certified check (made payable to "City of Youngstown"), or charge card. Self addressed stamped envelope.
In person – Hours are Monday - Friday, 8:00 a.m. - 4:00 p.m.
Electronic – VitalChek website 24 hours a day, 7days a week

Acknowledgement of Paternity Affidavit and Declaration of Paternity forms are prepared at no charge.
Each month, all original birth, death and fetal death certificates are forwarded to the, Ohio Department of Health, Division of Vital Statistics where they are permanently filed. A record of these certificates is kept on file in the Health District’s Vital Statistics Division.
